Kaletra is an oral medication that contains lopinavir and ritonavir. It is used for treating infections with the human immunodeficiency virus (HIV).

Indications

This drug is used to treat HIV-infected patients, in combination with other antiretroviral drugs.

Dosage and administration

Take the drug orally, regardless of meals. The tablet must be swallowed whole without chewing. Therapeutic dose – 2 tablets twice a day, with equal intervals between doses.

Contraindications

Severe liver failure; the simultaneous use of astemizole, blonanserin, terfenadine, midazolam (when taken orally), triazolam, cisapride, pimozide, salmeterol, sildenafil, tadalafil, vardenafil, avanafil, ergot alkaloids, HMG-CoA reductase inhibitors (lovastatin, simvastatin, atorvastatin), fosamprenavir, alfuzosin, fusidic acid, quetiapine; simultaneous use with St. John’s wort, boceprevir, simeprevir; simultaneous use with ketoconazole and itraconazole in high doses; concomitant use of a standard dose of rifampicin combination; simultaneous use of a combination and tipranavir with a low dose of ritonavir; children’s age up to 6 months; children’s age up to 3 years (for dosage forms intended for children older than 3 years and adults); the use 1 time / day in combination with carbamazepine, phenobarbital or phenytoin; the use oin combination with efavirenz, nevirapine, amprenavir or nelfinavir; use in children and adolescents under 18 years of age; use during pregnancy; hypersensitivity to lopinavir and / or ritonavir.

Restrictions on use

Viral hepatitis B and C; cirrhosis of the liver; mild and moderate liver failure; increased activity of liver enzymes; pancreatitis hemophilia A and B; dyslipidemia (hypercholesterolemia, hypertriglyceridemia); patients over 65 years; patients with organic heart diseases, patients with a history of cardiac conduction system disorders, or patients taking drugs that prolong the PR interval (verapamil or atazanavir), or drugs that extend the QT interval (phenyramine, quinidine, erythromycin, clarithromycin); simultaneous use with drugs for the treatment of erectile dysfunction, such as sildenafil, tadalafil; concomitant use with fentanyl, rosuvastatin, bupropion, inhaled or nasally administered GCS (e.g. fluticasone, budesonide), with antiarrhythmic drugs (such as bepridil, lidocainomy and quinidine), with digoxin, lamotrigine, valproic acid, thrazodiv, and bedacovide.

Side effects

The use of Kaletra can cause abdominal pain, gastroenteritis, gastritis, hemorrhagic colitis, dysphagia, diarrhea, dyspepsia, constipation, jaundice, flatulence, belching, fecal incontinence, nausea, sialadenitis, vomiting, stomatitis, pancreatitis, cholangitis, dry mouth, enteritis, esophagitis and enterocolitis.

In can also cause amnesia, ataxia, apathy, anxiety, psychomotor agitation, insomnia, headache, impaired orientation and thinking processes, depression, dizziness, dyskinesia, migraine, stroke, taste disturbance, unpleasant dreams, neuropathy, nervousness, peripheral neuritis, paresthesia, mental and sleep disturbances, convulsions, drowsiness, tremor, encephalopathy.

The use of the drug can lead to changes in vision, otitis media, tinnitus, varicose veins, vasculitis, atrial fibrillation, increased blood pressure, postural hypotension, tachycardia, deep vein thrombosis, thrombophlebitis, bronchospasm, bronchitis, shortness of breath, pulmonary edema, rhinitis, sinusitis, pharyngitis, anemia, lymphadenopathy, leukopenia, neutropenia, gynecomastia, thrombocytopenia, hypothyroidism, impaired glucose tolerance, male hypogonadism, diabetes mellitus, malabsorption of vitamins, Cushing’s syndrome, anorexia, dehydration, acidosis, obesity or weight loss decreased appetite, arthralgia, back pain, arthrosis, myalgia, impaired urination, bone necrosis, impaired ejaculation, nephrourolithiasis, decreased libido.

Allergic reactions may occur in the form of: acne, changes in the structure of nails, benign skin tumors, changes in skin color, skin rash, skin ulcers, lipodystrophy, itching of the skin, baldness, maculopapular rash, eczema, sweating, seborrhea, furunculosis, dry skin, exfoliative dermatitis . Asthenia, flu-like syndrome, chest pains, chest pains, weakness, fever, swelling of the face, chills, peripheral edema may also occur.

It can also change laboratory indicators: increase total cholesterol; change the content of bilirubin, glucose, uric acid, triglycerides in blood plasma; change the content of phosphorus and sodium in blood serum.

Avoid taking Kaletra with cobicistat, fosamprenavir, tipranavir, orlistat, boceprevir, rifampin, St. John’s wort, alfuzosin, tamsulosin, fluticasone, salmeterol, cisapride, sildenafil, tadalafil, ergotamine, dihydroergotamine, pimozide, rivaroxaban, simeprevir, telaprevir, midazolam, triazolam, lovastatin, simvastatin, hormonal birth control.

Pregnancy and breastfeeding

Kaletra can be used in the treatment of pregnant women with caution, in case of urgent need.

If a woman needs to take this drug, she should stop breastfeeding.

Storage conditions

Tablets should be stored in its original packaging at room temperature, away from heat, light, and moisture.
